Fill in what you need. The preview updates as you type.

Printed in a box at the top with the date and your name.

One question per line. Put choices after | and they print as check boxes. A line with no | gets two answer lines. Write (check all that apply) in the question for a multiple-answer item.

What you type is not saved on our server.

Keep it anonymous when you can

Answers are more honest when the form has no name on it. If you need contact details for a follow-up or a prize draw, ask for them in a separate last question and say what you will do with them.

How to use it

Type one question per line. For a multiple-choice question put the choices after a vertical bar: How often do you visit? | Daily | Weekly | Monthly. Each choice prints with a check box. A line without choices becomes an open question with two lines to write on. For a check-all-that-apply question, say so in the question text.

The introduction prints in a box at the top with the date and your organization: say why you are asking, how long it takes and how you will use the answers. A comments box and a thank-you line close the page.

Things to watch

One idea per question. Choices should not overlap (1 to 5 and 5 to 10 both contain 5) and should cover every case, so add Other or Not applicable when needed. Pilot the form on two or three people before you print fifty copies.

Frequently asked questions

How do I make a 1 to 5 rating question?

Write the five labels as choices: Very dissatisfied | Dissatisfied | Neutral | Satisfied | Very satisfied. They print in a row with check boxes.

Can I collect the answers online?

This template makes a printed form. Use it to settle the wording, then paste the same questions into Google Forms or a similar tool for an online version.

How many questions fit on one page?

About eight to ten multiple-choice questions with the intro box, or fewer if several are open questions. The preview shows the page break.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
