A plan for internal review
This is a plan for your own approval chain. Training that a regulator requires, or that is funded by a state workforce grant, usually has its own forms and hour requirements, so check those first.

How to use it
The plan tells the approver who is being trained, on what, when, and for how much. Write the goals as what people can do afterward, not what will be presented. Sessions go in one per line as date | time | topic | trainer and become a table; split a day into two lines when the morning and afternoon differ.
For the budget, put the basis next to each amount (hours times rate, copies times price). Type the headcount and the total row shows the cost per person. For checking results, list more than a survey if you can: a practice assessment or a manager check-in after 30 days shows whether the training stuck.
Things to watch
If the training is required by a regulator or paid for with a grant, the agency's own forms and hour counts come first. Use this plan to organize the sessions and budget, then transfer the details into those forms.

Frequently asked questions
How is the cost per person calculated?
The budget total divided by the headcount, rounded to the cent. Leave the headcount empty and the line is not shown.
What do I write after the training?
Use the training log for each session (what was covered, who attended, feedback). For a large program, add a status report with what was spent against this budget.
How many sign-off boxes do I need?
Match your company's approval chain. A small team often uses one box for the manager. Larger companies add Reviewed by and Approved by. Pick None if approvals happen by email and you only need the body of the form.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
