Required safety training has its own records
Training that OSHA or a state agency requires (forklift, hazard communication, bloodborne pathogens and similar) has specific recordkeeping rules, including what must be kept and for how long. Use this log for company training records and keep the required certificates and rosters alongside it.

How to use it
Fill the log in on the day of the session. Type the session name, date, time, hours, location and trainer. Paste the attendee names separated by commas and the form counts them and gives each one a row with a signature cell, so you can print the log and pass it around.
List the topics one per line so the same session can be run again from the log. In results and feedback, put the survey number first, then what worked and what to change. The sign-off box is off by default; turn it on when a manager reviews training records.
Things to watch
If the session is required by OSHA or a state rule, this log alone may not satisfy the recordkeeping requirement. Keep the trainer's certificate, the signed roster and any test results with it, and check how long the rule says to keep them.

Frequently asked questions
Can I use it for online training?
Yes. Type the platform in the location box. If the platform records attendance, save that export with the log instead of collecting signatures.
How are the hours used?
The hours box is the length of the session per attendee. It prints in the header so you can add up training hours per person across logs.
How many sign-off boxes do I need?
Match your company's approval chain. A small team often uses one box for the manager. Larger companies add Reviewed by and Approved by. Pick None if approvals happen by email and you only need the body of the form.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
