Ask for what the classroom needs

The form leaves out the full date of birth, home address and Social Security number on purpose: the school office already has them and a one-page sheet in a teacher's binder is not the place. Keep the returned forms in a closed folder and shred them at the end of the year.

How to use it

Most teachers print this blank and send it home on the first day, so you can leave the fields empty and print; every cell gets lines to write on. If you are a parent filling it in for your child, type the answers and the sheet prints ready to sign. Parents or guardians take one line each with name, relationship, phone, email and whether the student lives with them; emergency contacts take name, relationship and phone.

The pickup list, the medical notes and the "anything the teacher should know" box are the three cells teachers read first, so give them the most room. The signature line at the bottom confirms the information is current.

Things to watch

Keep the returned sheets in a folder you can grab on the way out during a fire drill: the emergency contacts and pickup names are what you need when a parent cannot be reached. Once the sheets are in, copy the names and phone numbers into the class roster template so the information is also in a file you can sort and print.

Frequently asked questions

Why month and day only for the birthday?

The class needs the day to celebrate; the year is identifying information the school office already has. A name, a full birth date and a parent's phone on one sheet is more than a classroom form should carry.

Where does the home address go?

Nowhere on this sheet. The office keeps the address in the student information system. If your school wants it on the classroom form, download the DOCX and add a row.

Can I collect it online instead?

You can type the questions into a form tool, but many families still prefer paper on day one. Paper also avoids storing health notes in a third-party account; check your district's rules before you put medical information in an online form.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
