Use your district's form when it has one

This is a general template, not legal advice. Many districts require their own permission and medical release form for off-campus trips. This slip covers the details families need and a basic medical consent; it has no liability waiver on purpose.

1 / 1

How to use it

Type the destination, date, times, cost and how the class travels. The letter writes its own opening paragraph from those details, or you can replace it with your own. Under the letter the details print as a small table, and below a cut line sits the slip families sign and send back.

The slip asks for an emergency contact, allergies and medications, and a short consent for staff to arrange medical care. Set a return-by date so the bus count is firm a week before the trip.

Things to watch

Keep the whole page to one sheet so the slip stays attached to the details. Collect the returned slips in a folder and bring them on the trip; the emergency contact and medication lines are what you need if something happens.

Frequently asked questions

Why is there no liability waiver?

Waivers are legal documents that vary by state and district, and courts often read them narrowly. If your district wants one, it will have its own wording. This slip sticks to permission, emergency contact and medical consent.

Can I use it for a club or sports trip?

Yes. Put the club name on the letterhead line and describe the event in the destination and times boxes. The reply slip works the same way.

How do I send it in Spanish too?

Make a second copy with translated text in the same boxes and print both. Many districts require translations for families who ask.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
