This is a sample form. Check that the terms and pay match federal and state labor rules and your company policy.

Only for pay that was actually made

A pay stub records real wages paid to a real employee. Making one for pay that was never made, or changing the numbers on a real one, and handing it to a lender is a federal crime. Using it to rent a home or get other credit is fraud under state law.

Check your state's pay stub rules

This is a general template, not legal advice. Many states require a pay stub with specific items such as hours, rates and deductions, so check your state labor department.

How to use it

Fill in the employer, a payroll contact, the employee and the pay period. Add one earnings line per pay type: hours and rate for hourly pay, or just the amount for salary, tips or a bonus. Then type each deduction from your payroll records as name, this period and year to date. The stub adds up gross pay, deductions and net pay for the period and the year.

Print it or download the DOCX and give one copy to the employee with the paycheck. Keep a copy with your payroll records.

Things to watch

This template does not calculate federal or state withholding. Use the IRS withholding tables in Publication 15-T or your payroll software, then copy the amounts here. Social Security is 6.2% and Medicare 1.45% of the employee's taxable wages, but pre-tax benefit deductions can change what counts as taxable.

Show only the last 4 digits of the Social Security number or an employee ID. The footer always prints the employer's contact so a landlord or lender can check the stub.

Frequently asked questions

Do I have to give employees a pay stub?

Federal law does not require one, but many states do, and some list exactly what it must show, such as hours worked, pay rates, each deduction and the pay period dates. Check your state labor department before you rely on this layout.

Can it calculate taxes for me?

No. Withholding depends on the employee's W-4, the state and the benefits they have, so type the amounts from IRS Publication 15-T, your state's tables or your payroll software. The stub only adds up what you enter. For a whole team, the payroll register spreadsheet does the Social Security and Medicare math.

Can I make a pay stub for myself to show a landlord?

Only if it records pay you really received, for example as an owner paying yourself through payroll. A stub for income that was not paid is fraud, and the footer lists the employer's contact so the landlord can verify it. Self-employed people usually show tax returns, 1099 forms or bank statements instead.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
