Parent-Teacher Conference Form
| Student | Jordan Lee | Grade or class | Grade 3, Room 12 |
|---|---|---|---|
| Teacher | Taylor Kim | Date | October 22, 2026 |
| Attendees | Casey Lee (mother), Taylor Kim (teacher) | ||
| Strengths (glow) | Jordan reads above grade level and picks chapter books on his own. He is kind to classmates and is the first to help someone who is stuck. His science notebook is the most detailed in the class. |
|---|---|
| Areas to work on (grow) | Multiplication facts through 10 are still slow, which makes the two-step word problems hard. Jordan rushes written work and skips the checking step, so small errors stay in. |
Academic progress by subject
| Subject | Current grade | Comments |
|---|---|---|
| Reading and language arts | A | Reads at a Grade 5 level. Writing is strong on ideas, needs work on paragraph breaks. |
| Math | B- | Understands the concepts; fact fluency slows him down on timed work. |
| Science | A | Curious and careful; asks good questions during experiments. |
| Social studies | B+ | Participates well. Map quiz was the one low score this quarter. |
| Behavior and social | Follows directions and works well in groups. Gets distracted when seated near the window; moving his seat last month helped. |
|---|---|
| Goals until the next conference | 1. Know multiplication facts 0 to 10 with no more than 3 seconds per fact by the December conference. 2. Use the two-minute check at the end of every math page. |
| How parents can help at home | 1. Five minutes of flash cards or a fact app on school nights, not weekends. 2. Ask Jordan to read his math answers out loud before he puts the sheet in his folder. 3. Keep reading together; it is working. |
| Next steps and follow-up date | 1. Teacher sends home a fact fluency chart every Friday. 2. Follow-up check-in by phone the week of November 16. 3. Next conference: December 10, 2026. |

How to use it
Fill in the student, the date and who will be at the table, then write the glow and grow boxes before the conference: strengths first, then one or two areas to work on with an example from the student's work. The subject table takes one line per subject with the current grade and a short comment. Leave the behavior, goals, home help and next steps boxes short so there is room to add what the parent says during the meeting.
Print one copy per family. Both of you sign at the bottom, the parent takes the form home and you keep a photocopy or a scan in the student's file. Empty boxes print with lines so you can write in by hand.
Things to watch
Two goals are more likely to happen than five. Pick the ones that matter most until the next conference and write how you will both know they were met. The follow-up date on the form is what turns a good conversation into a plan; for the end-of-year record, the student information sheet and class roster templates hold the contact details you will need.

Frequently asked questions
What are glow and grow?
A common conference format: glow is what the student does well, grow is what to work on next. Parents hear the strengths first, which makes the second box easier to take in.
Can I change the subjects?
Yes. Type one line per subject with the grade and a comment. Specials, electives or a single subject for a middle school teacher all work; the table prints whatever lines you type.
Does the parent need to sign?
Most schools ask for it so the file shows the conference happened and what was agreed. If a parent cannot attend, hold the conference by phone and send the form home for a signature.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
