A permission slip may need your district's form

For field trips and activities with any risk, most districts require their own permission and medical release form. Use this letter to explain the event and attach the district form, or use the reply slip here only for simple RSVPs.

1 / 1

How to use it

Write the letter in two or three short paragraphs: what is happening, what you need from families, and when. Then put the facts in the what, when, where, cost and what to bring boxes. They print as a small table under the letter, so a parent skimming on a phone photo of the page still gets the details.

Turn on the reply slip when you need something back. It prints below a cut line with the student name, a yes or no check box, a notes line and a signature. Finish the yes sentence in the box provided, for example "may attend the field trip on October 29".

Things to watch

Keep the letter to one page so the slip is on the same sheet. Ask only for what the event needs on the slip; health details and emergency contacts belong on the district form, not on a tear-off that sits in a backpack.

Frequently asked questions

Can I send it without a reply slip?

Yes. Turn the reply slip off and the page ends at your signature. That works for newsletters and general announcements.

How do I address it to one family?

This template is written for the whole class, so it opens with 'Dear families'. For one family, download the DOCX and change the greeting, or use the business letter template.

Where does a Spanish version go?

Make a second copy of the letter with the translated text in the same boxes and print both. Many districts require translations for families who ask for them.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
