This is a sample form. Check that the terms and pay match federal and state labor rules and your company policy.

Fill in what you need. The preview updates as you type.

Printed in the Position applied for box. Leave empty for a general application.

Only questions you are allowed to ask

This is a general template, not legal advice. The form leaves out Social Security number, date of birth, marital status, citizenship beyond work authorization, criminal history and a background check consent on purpose. Many states and cities limit those questions before a conditional offer (ban the box laws, salary history bans). Collect them later on the forms built for them.

How to use it

Type your company name and, if you are hiring for one role, the position. Choose two or three employment blocks and whether you want a references table, then print. The applicant fills the form by hand: contact details, availability, education, work history with a may-we-contact box for each employer, and a signed certification at the end. It runs to two Letter pages.

Keep a stack at the front desk or attach the PDF to a job post. When a candidate is hired, move on to the new hire checklist, the W-4 and I-9, the direct deposit form and the emergency contact form; this application does not collect that information.

Things to watch

Do not add questions about age, marital status, children, religion, national origin or arrests. Several states also ban asking about salary history, so the form asks for desired pay instead. If you run background checks, use a separate consent form that follows the Fair Credit Reporting Act, given after a conditional offer where your state requires it.

Frequently asked questions

Why is there no Social Security number field?

You do not need it until you hire the person, and a stack of applications is a poor place to keep it. Collect it on the W-4 and I-9 and store those in a restricted folder.

Can I ask whether the applicant has been convicted of a crime?

Not on this form. Many states and cities only allow the question after a conditional offer, and some restrict it entirely. If your state allows it and the job requires it, ask on a separate form at that stage.

Can the applicant fill it in on a computer?

Download the DOCX and the boxes can be typed into in Word. The printed form is designed for handwriting, so the boxes are generous.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
