Specific beats modest
A self-evaluation is not the place to be vague. Name the project, the number and the date. For improvements, pair each one with what you will do differently, so the section reads as a plan rather than a confession.

| 3. What I would improve | I tend to hold work until it is polished, which delayed the Q2 landing page review by a week. I want to share drafts earlier and ask for feedback at the halfway point. My presentations to the sales team run long; I will cut them to ten minutes with a one-page summary. |
| 4. Goals for the next period | Own the paid social channel end to end by the end of Q2, including budget and reporting. Complete the GA4 advanced certification by March. Mentor the new associate joining in January through their first 90 days. |
| 5. Support I need | A half day a week in Q1 to build the social reporting, and a seat in the leadership presentation workshop in February. |
This is my own assessment of my work during the review period. I understand my manager will add their review and we will discuss both. December 4, 2026 Employee signature: Jordan Lee Manager (received): Taylor Kim, Director of Marketing | |
How to use it
Fill in the header and write your achievements, improvements, goals and the support you need, one item per line. The form prints the six standard areas (or your own list) with a 1 to 5 check line and room for a comment under each, which you fill in by hand or in the DOCX. Sign and hand it to your manager before the review meeting.
The areas match the employee evaluation form, so your manager can set the two side by side and talk about where the ratings differ. Bring last period's goals to the meeting and say which ones you met.
Things to watch
Rate honestly: a row of fives invites a harder conversation than a four with a good example. Keep the improvement section to one or two items you can act on. If you are asking for a raise or a promotion, that belongs in the meeting and in the goals, not in the ratings.

Frequently asked questions
Can I type the ratings instead of checking boxes by hand?
Download the DOCX and replace the boxes in Word, or type the rating at the start of the comment line. The printed form is designed to be checked with a pen.
What if my manager's ratings are lower than mine?
That is what the meeting is for. Ask for the example behind each lower rating and compare it with yours. Write down what a higher rating would look like next period and put it in your goals.
How long should it be?
Two pages. Three achievements with numbers, two improvements, three goals and a sentence on support is enough for most roles.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
