An internal review tool, not a legal form
Use it with your own review policy. If ratings feed pay or promotion decisions, write down the rule in advance and keep the comments specific enough to explain the result.

This shows the first part of each sheet as it prints on Letter paper. The file also has a how-to sheet and formulas, and every column fits one page width when you print from Excel.
How to use it
Type the review period, choose how many forms you need and download. Each form is one sheet: the employee block at the top, eight competencies rated 1 to 5 with weights you can change, goals for the next period, comments and signature lines. It prints on one Letter page.
Rate each competency from the dropdown and the overall score and rating fill in. The rating scale is a small table of yellow cells, so you can rename the labels or move the cutoffs to match your policy.
Things to watch
Write comments that point to specific work, not personality. If the ratings feed raises or promotions, decide the rule before the reviews start and apply it to everyone the same way. The employee's signature only confirms the review was discussed, so say that out loud when you hand the form over.

Frequently asked questions
Can I change the competencies?
Yes. Type over the names and descriptions in section 2 and adjust the weights. Adding rows means extending the formulas in the overall score, so start with renaming.
What if I leave a rating blank?
The row is left out. The overall score divides by the weights of the rated rows only, so one blank does not drag the score down.
How do I change the rating labels?
Edit the yellow cells in section 3. Keep the minimum scores in ascending order from top to bottom.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
