How to use it
Type your name and, if you are tracking one thing such as migraines or joint pain, its name; the tracker then drops the symptom column and gives the space to triggers and what helped. Print it and write one row per episode: when it started, how bad it was on a 1 to 10 scale, how long it lasted, what you think set it off and what you tried.
The box at the bottom collects questions as they come to you, so you do not forget them in the exam room. Bring the sheet to the visit; a doctor can see frequency, severity and triggers at a glance instead of asking you to remember.
Things to watch
Write the episode down the same day. Severity scored a week later drifts toward the middle. Keep the trigger column honest even when the answer is embarrassing (three coffees, four hours of sleep), because that is usually the useful part. A sudden severe symptom you have never had before is a call to the doctor or 911, not a row on the sheet.

Frequently asked questions
What counts as duration?
From when you noticed the symptom until it was gone or back to its usual level. Write 4 hours, 2 days, or ongoing if it has not stopped.
Can I track more than one symptom?
Yes. Leave the symptom box empty and the Symptom column prints, so each row can be a different thing. If two symptoms have different triggers, two separate sheets are easier to read.
Which log goes with it?
The food diary for the same days if you suspect a food trigger, the sleep diary if the symptom follows bad nights, or the blood pressure log if it is headaches or dizziness.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
