A general form, not legal advice

This is a general template, not legal advice. The form leaves out date of birth, Social Security number and every Fair Housing protected question on purpose. Collect the Social Security number and the credit and background check authorization on a separate screening consent form, and apply the same criteria to every applicant.

  • Several states and cities cap application fees or require a receipt and an itemized list of screening costs. Check the rule where the property is.

How to use it

Type the property, the unit, the rent and the lease term, and decide whether you want a co-applicant section and the vehicles and pets tables. Print a stack or save the PDF and hand it to anyone who asks to apply. The applicant fills in the rest by hand: contact details, current and previous address with the landlord's phone number, employer and income, references and an emergency contact, then signs the statement at the end.

Each adult who will be on the lease completes their own application, or the co-applicant section when they apply together. When the forms come back, call the previous landlord and the employer; the statement on the form is the applicant's permission to do that.

Things to watch

The Fair Housing Act and most state laws forbid asking about race, color, religion, national origin, sex, familial status or disability, and many states add source of income, age and marital status. This form does not ask about any of them, and you should not add them. Application fees are capped or regulated in a growing number of places; the preview reminds you when you type one.

Frequently asked questions

Where does the Social Security number go?

Not on this form. Collect it on a separate screening consent form that authorizes the credit and background check, and keep that form in a locked file or an encrypted folder. A general application gets copied and emailed too often to hold it.

Can I ask how many people will live in the unit?

Yes. The number of occupants is a normal occupancy question and the form asks for it. What you cannot ask is whether they are children, whether the applicants are married, or anything that reveals familial status, disability or another protected class.

What happens after the application?

Check the references and income, run the screening with the applicant's separate written consent, and apply the same criteria to everyone. Then send an approval or a denial in writing; if a credit report drove the denial, the Fair Credit Reporting Act requires an adverse action notice naming the reporting agency. The next step for an approved applicant is the lease, and the move-in move-out checklist on move-in day.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
