A general template, not legal advice
This is a general template, not legal advice. A release is a contract; if the recordings will be used in paid advertising or for a public figure, have a lawyer check the wording and the state's right-of-publicity rules.

I, Jordan Lee, give Acme Inc. permission to use photographs, video and audio recordings of me taken at the Acme community volunteer day at Zilker Park on October 3, 2026, including my name, likeness and voice, for the uses checked above. Acme Inc. may edit, crop, caption and combine the recordings with other material and may reproduce them in any media now known or later developed, for the uses checked above.
This release has no time limit. I am not receiving any payment for this release, and I will not claim any payment, royalty or right to inspect or approve the finished material. I release Acme Inc. from any claim arising from the use of the recordings as permitted here, including claims of invasion of privacy or defamation, unless the use is outside what this form allows.
I may revoke this release for future uses by written notice to Acme Inc. Material already published or in production when the notice is received may remain in use.
I am 18 or older and I have read and understood this release.

How to use it
Type the name and contact of the person in the photos, who the release goes to, and the event or occasion where the photos were taken and when. Check the uses you agree to; only the checked ones are granted. Pick a term: no time limit, which is what most organizations ask for, or an end date. For a child, turn on the minor toggle, add the child's name, and the parent or guardian signs.
Print one copy per person. The signer signs and dates at the bottom, and you keep the signed form with the photos so you can show permission later. Turn on the notary block only when the organization asks for it.
Things to watch
A release covers what it names. If you later want to use the photos in a paid advertisement and Advertising is not checked, ask again. The revocation paragraph is for future uses only; it does not oblige the organization to take down material already published. For employees, volunteers and students, collect the form once at the start of the year or the event rather than after the photos exist.

Frequently asked questions
Do I need a release for photos taken in public?
For news and editorial use, usually not. For advertising, promotion or anything that implies the person endorses something, yes, because most states give people a right to control commercial use of their likeness. When in doubt, get the form signed.
What is the difference between a photo release and a model release?
Very little. A model release is the same document used by photographers for paid or unpaid models, often with a payment clause. This form says no payment; if you are paying the subject, add the amount to the description or use the photographer's contract.
Does this have to be notarized?
Not by law in most cases, but many schools, airlines, hospitals and border officers will not accept a consent form without a notary stamp. A notary at a bank, shipping store or online notary service costs a few dollars. Sign in front of the notary, not before.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
