An order form is not a receipt
It records what was ordered, where it ships and how the customer intends to pay. Give the customer a receipt once payment clears. Shipping and billing addresses are personal data, so share the form only with the people who need it.

How to use it
Type your business as the seller and the customer's name, contact and billing address. Add a separate ship-to address only when it differs. Then enter each item with the option or size, a quantity and the unit price. The form totals the items, adds sales tax at the rate you type and adds a flat shipping charge if you give one.
Pick the shipping method and the payment method from the lists. The payment method prints as a row of check boxes with the chosen one filled, so a printed blank form still works for walk-in customers.
Things to watch
Whether you charge sales tax on shipping depends on your state. This form leaves shipping untaxed. If your state taxes it, add the shipping as a line item instead so tax applies to it.
Keep the order number on the packing slip and the receipt so you can trace one order through all three documents.

Can the customer fill this in by hand?
Yes. Leave the customer fields and items empty, print the PDF, and the form prints with blank rows and the full row of payment check boxes.
How is the total calculated?
Items are quantity times unit price. Sales tax applies to the item subtotal at the rate you type. The shipping charge is added after tax. Leave shipping at 0 to hide that line.
What is the Excel version for?
Repeat orders from the same customer. Download it once, then change the yellow cells. The amount, subtotal, tax and total are formulas.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
