How to use it

Type your name, the first day and what you are trying to learn, such as a food that sets off a symptom or where the extra snacks come in. The diary prints as a week table with tall cells for breakfast, lunch, dinner and snacks, a water count and a column for how you felt. Turn on the calories column if your dietitian asked for numbers.

Keep the sheet in the kitchen or fold it into a bag and write after each meal, not at the end of the day. Three honest days say more than a week filled in from memory. Bring it to the appointment with the blood sugar or symptom log for the same dates.

Things to watch

Write what you actually ate, including the bite of someone else's dessert; the diary is for you, not for a grade. Portions can be rough (a bowl, a handful, a 12 oz can). If you are tracking for a food allergy or intolerance, note the time symptoms began next to the meal so the gap is visible.

Frequently asked questions

Do I need to count calories?

Only if your plan asks for it. Most elimination diets and symptom hunts need the what and when, not the numbers. Turn the column on when a dietitian or a weight loss program wants a total per day.

How many glasses of water?

Write the count you drank. The log does not set a target because needs vary with size, climate and activity; your doctor or dietitian can tell you what to aim for.

What goes with it?

The weight tracker spreadsheet for weekly weigh-ins, or the blood sugar log if you test. Matching dates on both sheets is what makes the pattern visible.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
