First line the street address, second line cross streets, gate code or landmarks for the ambulance. Prints large at the top.

One person per line. Date of birth is optional; paramedics ask for it.

Parents at work first, then a neighbor with a key and an out-of-town relative.

Write where the card is rather than the full member number if the sheet hangs where visitors see it.

How to use it

Type your address with the cross streets or landmarks an ambulance would need, who lives in the house with any allergies, and the people to call. Add doctors, the school, utilities and insurance as one line each. The sheet prints on one page with the address, Poison Control and the 911 line in large type at the top, so anyone in the house can read it from across the kitchen.

Put one on the fridge and one inside the front hall closet. Give a copy to the babysitter with the babysitter information sheet and photograph it for your phone. Check it when the clocks change.

Things to watch

Children and visitors should be able to use it, so the address line comes first and is written the way you would say it on the phone. Keep account and policy numbers short or point to where the card is, because the sheet hangs where guests see it. If someone in the house has a severe allergy or a condition like epilepsy, put where the medication is in the same cell.

Frequently asked questions

Why is the address on an emergency sheet?

In a panic people forget it, and a babysitter or guest may never have known it. Mobile 911 calls do not always give the dispatcher an exact location, so the caller reads it from the sheet.

Should I put dates of birth on it?

Optional. Paramedics ask for age or birth date, and for a child it decides medication doses. Leave it off if the sheet hangs where many visitors pass.

What about a wallet card?

Fold a copy and keep it in the glove box or with your child's school papers. For a sitter, the babysitter information sheet template has the same emergency block plus bedtimes and house rules.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
