A receipt, not tax advice

This receipt records what the provider received. Whether the parent can claim the child and dependent care credit, and how much, depends on their return. The tax ID stays in your browser and is not sent anywhere; it is left out of shared links.

How to use it

Type the provider's name, address, phone and tax ID, the parent, the children, the period the payment covers and the amount. Pick how it was paid and the date, then print and sign. The receipt puts the amount in a bold line at the top and the provider's tax details where the parent can find them at tax time.

At the end of the year, switch on the year-end statement: the title changes, the period becomes the tax year and the amount is the total paid. One statement per family is what most parents ask for in January.

Things to watch

Parents claim the child and dependent care credit on Form 2441, which asks for the provider's name, address and tax ID, the same items as Form W-10. A provider who runs a business usually has an EIN; a nanny or babysitter paid directly may give an SSN. Keep a copy of every receipt and log the payments in the rent ledger or a simple income ledger so the year-end total adds up.

Frequently asked questions

Do I have to give my SSN?

The parent needs a tax ID for the credit, and a provider who refuses can be reported to the IRS on Form 2441. If you do not want to give your SSN, get a free EIN from the IRS site and use that on every receipt.

Can a babysitter or nanny use this?

Yes. Type your own name as the provider and your home address. If the family pays you through payroll as a household employee, their W-2 already carries the numbers and this receipt is a courtesy copy.

What if the family paid for several months at once?

Make the period of care cover all the months paid and type the whole amount. For a year-end summary, switch on the year-end statement and type the total for the year.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
