A general template, not legal advice

This is a general template, not legal advice. Some hospitals and states require a notarized form or their own consent form, and a few states have a statutory caregiver authorization affidavit. Call the pediatrician's office and the nearest hospital to ask what they accept.

How to use it

Type the child's name and date of birth, then allergies, medications and conditions one per line, because that is what an emergency room reads first. Put each parent on a line with name, relationship and phone, and the caregiver on four lines. Pick a period, check what the caregiver may consent to, and add the insurance, the pediatrician and the hospital you prefer. The consent paragraph is written from those boxes.

Print two copies. Both parents sign and date, the caregiver signs to accept, and you sign in front of a notary so a hospital does not question it. The caregiver keeps one copy with a photo of the child's insurance card; you keep the other.

Things to watch

Surgery is checked with the condition that providers try to reach a parent first; keep your phone on. If the child has a condition that needs a specific plan (seizures, diabetes, severe allergies), attach the doctor's action plan to this form. Renew the form when the caregiver, the medications or the insurance change.

Frequently asked questions

Who needs this form?

Anyone who looks after your child when you are not reachable: grandparents during a visit, a sitter for a weekend away, a relative the child stays with for a school term, the adult who takes the child on a trip. Hospitals treat true emergencies without consent, but routine and urgent care often waits for a parent unless a form like this exists.

Can I name more than one caregiver?

Yes. Put each on their own lines in the caregiver box, or print one form per caregiver so each has an original to show. Only the first line is used in the consent sentence, so list the main caregiver first.

Does this have to be notarized?

Not by law in most cases, but many schools, airlines, hospitals and border officers will not accept a consent form without a notary stamp. A notary at a bank, shipping store or online notary service costs a few dollars. Sign in front of the notary, not before.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
