Approval steps and required fields vary by employer. If your company has its own form, use that one and treat this as a quick way to draft the same information.

This shows the first part of each sheet as it prints on Letter paper. The file also has a how-to sheet and formulas, and every column fits one page width when you print from Excel.
How to use it
Pick how many sign-off rows you need and whether you want a Pay by column. The sheet has the usual header fields, ten expense lines with dropdowns for the account code and the backup document, a total, and room for attachments and notes. The sign-off rows sit at the bottom with the role name in a yellow cell you can rename.
Write the description so the approver can tell what was bought and why without opening the attachment. Staple or scan the invoice or receipt behind the printed sheet, or list the file names in the Attachments cell.
Things to watch
Account codes here are general groupings. If your company has a chart of accounts, use those names so finance can post the payment without recoding it. The page is set to print on one Letter sheet in portrait, so if you add many lines check the print preview before you print.

Frequently asked questions
Can I change the sign-off roles later?
Yes. The role labels are plain yellow cells, so type over them. To change how many rows there are, pick a different count here and download again.
Which account code do I pick?
Go by what the money was for. Client lunch is Meals and entertainment, a toner cartridge is Office supplies, a contractor invoice is Professional services. The Account guide sheet has one line per code. Your company chart of accounts wins if it differs.
What counts as backup?
An invoice or itemized receipt is the usual proof. A card statement shows the charge but not what was bought, so most finance teams want the receipt too. Pick None only when there is really nothing, and say why in Notes.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
