How to use it
Type your name, the first day of the week and, if your doctor gave you one, the target range in your own words. The page prints landscape with a row for each day and a box for each test time: before and after each meal, bedtime, the insulin or medication you took, and a notes box. Most people only fill the boxes their doctor asked for and leave the rest empty.
Pick two weeks when you want one sheet for the whole period between visits. Bring the sheet to the appointment or photograph it for the portal; a pattern across a week says more than any single number.
Things to watch
The log has no normal range printed on purpose. Targets differ by type of diabetes, age, pregnancy and medication, so write the range your doctor gave you. If a reading is far outside that range and you feel unwell, follow the plan your care team gave you for highs and lows rather than waiting for the next visit.

Frequently asked questions
Can I use mmol/L instead of mg/dL?
Yes. The column headers do not name the unit; only the subtitle says mg/dL. Cross it out or type your target range in mmol/L and the boxes work the same way.
Where do I write insulin doses?
In the Insulin or meds column for that day, for example 12 units at breakfast, 8 at dinner. If you dose at every meal, write the dose next to the reading in the same box.
What else helps my doctor?
A food diary for the same week. The food diary template prints in the same style, and the two sheets together show why an after-meal number was high.
Is what I type saved anywhere?
No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
