Fill in what you need. The preview updates as you type.

Type it the way your doctor gave it. Empty prints a blank line.

With a date, every row is pre-filled with its day. Empty leaves the date column blank.

What you type is not saved on our server.

Not medical advice

This log is for your own records and for your doctor. It does not diagnose anything or replace medical advice. If a reading worries you, call your doctor.

How to use it

Type your name, the first day and whether you take one or two readings a day. The log prints with a row for each reading, the date already written in, and the adult categories at the top so you can read a number against them without looking it up. The notes column is for what can move a reading: which arm, standing or seated, a missed pill, a stressful morning.

Keep the page next to the cuff. Two weeks of morning and evening readings is what most doctors ask for before changing a medication, so bring the sheet to the visit or photograph it and send it through the patient portal.

Things to watch

One high reading is not a diagnosis. Rest five minutes, take two readings a minute apart and write both. If you see 180 or higher, or 120 or higher on the bottom number, and you have chest pain, shortness of breath or trouble speaking, call 911 rather than writing it down.

Frequently asked questions

Which number goes where?

Systolic is the top, larger number the cuff shows; diastolic is the bottom one. Pulse is the heart rate, which most home cuffs show as a third number.

Should I log morning and evening?

Ask your doctor. Many hypertension plans want two readings in the morning before medication and two in the evening. Turn on the AM and PM rows for that, and write the second reading of each pair in the notes column.

What do I do with the log at the visit?

Hand it over or read the averages out. Use the symptom tracker template alongside it if headaches or dizziness are part of the picture.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
