This form says the receiver got the items listed, in the stated condition, on the stated date. Check each item before signing and write any shortage or damage in the condition column or the notes. For money, use a receipt instead.

How to use it

Pick what was handed over, type the date and who it came from, then list each item with a quantity and its condition. Use the condition column for anything you would want on record later: new in box, scratched, two pages missing. Serial numbers and asset tags go in the item description.

Type the name and title of the person receiving and the name of the person delivering. Both sign the printed copy; the statement above the signatures says exactly what is being confirmed.

Things to watch

Print two copies and have both signed, one for each side. The form only confirms receipt. It does not say the items were accepted as correct or that payment is owed, so keep a separate invoice or contract for that.

Frequently asked questions

When do I need an acknowledgment of receipt?

Whenever something of value changes hands without a sale: company equipment issued to an employee, original documents handed to a client, keys or badges returned, samples delivered to a customer.

What is the difference between this and a receipt?

A receipt confirms money was received. An acknowledgment of receipt confirms goods, equipment or documents were received, with their condition. If money changed hands as well, use the receipt template for that part.

Who keeps the signed copy?

The party that handed the items over keeps the original, since it protects them. The receiver keeps a copy so the two records match.

Is what I type saved anywhere?

No. Everything stays in your browser and nothing is sent to our server. If you refresh the page it is gone, so print, save a PDF or download the DOCX when you are done.
